A Big Worry for the State Department of Revenue

Each year, tens of thousands of payment checks are submitted to the state Department of Revenue without the necessary identifying information to readily identify the taxpayer or tax liability and post the payment to the proper account, sometimes leading to errors and delays. So, every year, the Department begs the public to use an appropriate payment voucher form and document the taxpayer(s) names and tax year on the check.
The Department really prefers the check to include the primary Social Security Number or Tax ID Number. If that is a security risk to you, then the last four digits of the SSN would still be helpful.
